This is a community based role involving regular travel across the Cardiff and Vale of Glamorgan area, including local meetings and direct support, with the flexibility to work from home based on business and community needs.
We are looking to welcome an approachable and knowledgeable Community Support Worker to work with people affected by Young Onset Dementia. This is an amazingly rewarding and varied opportunity to provide one to one support to individuals, enabling them to build confidence, maintain independence and engage in activities they enjoy.
This could be as simple as meeting for a coffee or visiting a local area of interest, through to supporting someone to access public transport, attend appointments, continue participating in community activities, maintain employment, or pursue hobbies and interests that are important to them.
We promote a person centred service, ensuring our support is tailored to each individual based on their own aspirations, strengths and needs. We provide information and support in people’s homes and at locations throughout the local community.
People living with Young Onset Dementia often face unique challenges, including managing work, family responsibilities, relationships and social connections. This role focuses on helping people live well with dementia and remain connected to the things that matter most to them.
The role is based within communities across Cardiff and the Vale of Glamorgan and you will be required to travel throughout this area. This is an essential requirement for the successful candidate.
