Working within local communities across Oxfordshire you will be joining our successful Community Information Network Team as we work to further improve access to information, support, activities and opportunities for all those with a health, care or support need as well as supporting those we work with to access the digital world and build their confidence in doing so. You will be working with individuals, families and groups both in the community and in their own homes as well as a network of local digital champion volunteers. We take a blended approach so you will be involved in different project streams within your role which might include working within the hospital setting, alongside adult social care or within GP surgeries but all are underpinned by our work with local communities to further build upon and support what is being offered as well as working with individuals and their families to connect them with the support and opportunities that exist, particularly with a focus on the digital world. We are passionate about supporting and empowering people so that they can maintain their independence, health, wellbeing and ‘live life to the full’
