Working within local communities across Oxfordshire you will be joining our successful Community Information Network Team as we work to further improve access to information, support, activities and opportunities for all those with a health, care or support need.
You will be working with individuals, families and groups both in the community and in their own homes. In addition, you may be working closely with local GP surgeries to provide the new social prescribing element into the support they offer to their patients or with the locality based Adult Social Care Teams.
You will also have the opportunity to join our hospital discharge support team working across various settings including the John Radcliffe, Horton Hospital and community hospitals. We take a blended approach so you will be involved in different project streams within your role but all are underpinned by our work with local communities to further build upon and support what is being offered as well as working with individuals and their families to connect them with the support and opportunities that exist.
